Athletic Arts Academy LLC requests your consent to photograph and record video of you or your child during classes, events, and performances. These photos and videos may be used to promote the Academy through various media outlets, including but not limited to our website, social media channels, newsletters, printed promotional materials, and other official Academy communications.

 

By signing this consent form, you authorize the Academy to use photos and videos in which you or your child appear, without any compensation.

 

Consent Terms

 

1. Consent to Use Photographs and Video Recordings  

I hereby authorize Athletic Arts Academy to photograph, video record, and otherwise capture visual and audio images of myself or my child during classes, events, and other Academy-related activities.

 

2. Usage Authorization

   I grant Athletic Arts Academy permission to use these photographs and video recordings in any lawful way for educational, promotional, advertising, and informational purposes. This includes but is not limited to use on:

   - The Academy's website

   - Social media platforms (e.g., Facebook, Instagram, YouTube)

   - Printed brochures, flyers, and newsletters

   - Posters and other promotional materials

 

3. Waiver of Rights

I understand that all photographs and recordings taken are the property of Athletic Arts Academy, and I waive any rights to inspect or approve the final products in which they are used.

 

4. Release from Liability

I release and discharge Athletic Arts Academy and its representatives from any claims or demands that may arise out of or in connection with the use of photographs and videos, including any claims for libel or invasion of privacy.

 

5. Duration of Consent

I understand that this consent is perpetual, meaning that it will remain in effect indefinitely unless I provide written notice to Athletic Arts Academy requesting revocation of my consent. This revocation will not affect any photographs or recordings already used or published before the request is received.

 

Acknowledgment and Agreement

 

I have read and understood the terms outlined above. By initialing below, I confirm my consent for Athletic Arts Academy LLC to use photographs and video recordings as described.

 

If the participant is under 18, please complete the following:

 

I am the parent or legal guardian of the participant named above. I have read and understood the terms of this release, and I consent to the use of photographs and videos of my child by Athletic Arts Academy.
